I, the undersigned, hereby give my consent to San Isidro College (SIC), located in Impalambong Malaybalay City, Bukidnon, to collect, process, and use my personal data and/or that of my child/ward in accordance with the Data Privacy Act of 2012 (RA 10173), its Implementing Rules and Regulations, and the school’s Data Privacy Notice.

I acknowledge that I have read and understood the SIC Data Privacy Notice and that I am fully informed of the following:

• The types of personal data that will be collected and processed by SIC.
• The purposes for which the data will be used, including but not limited to academic, administrative, statistical, and historical reasons.
• The storage, retention, and eventual disposal of my/my child’s/ward’s data.
• The sharing of personal data with authorized third parties in accordance with legal and institutional requirements.
• My rights as a data subject under the Data Privacy Act of 2012, including the right to be informed, access, correct, object, data portability, erasure/blocking, damages, file a complaint, and transmissibility.

I understand that I have the right to withhold consent, but acknowledge that doing so may affect my/my child’s/ward’s enrollment or participation in SIC’s academic and non-academic activities as required by the institution's legitimate interests and regulatory requirements.

By signing this consent form, I voluntarily and freely give SIC permission to process my/my child’s/ward’s personal data for the purposes stated above and in accordance with its Data Privacy Notice.
